Preprinting Star CBM Forms
When you use the Print or Mixed formats for assessments, you can either print the forms as you select the assessment format or use preprinted forms from the booklets linked below.
Printing the Forms
If you decide to use preprinted forms instead of printing forms for each assessment, you can print the booklets linked under "Student and Teacher Forms" below. Three things to note:
- For Rapid Color Naming and Rapid Picture Naming assessments, be sure to print the forms in color; other assessments can be printed in grayscale.
- For Phoneme Segmentation, there are no student forms since students listen and then answer verbally.
- If you use the Print format and you mark scoring information on the teacher form, you may need to print the teacher form each time.
Finding the Correct Form for Each Assessment
Students are randomly assigned specific forms for each assessment by the software. It's important to administer the form that has been assigned for each assessment; otherwise, you will be unable to score the form you administered because when you enter scores, you will be shown the form that was assigned.
To help you identify and find the form, Star CBM shows you the assigned form ID as described under "Mixed Format Assessments" and "Print Assessments" below. Note that the form identifiers include the measure abbreviations, which are:

For Passage Oral Reading assessments, the form ID also includes the grade level. For example, POR-G1: Form 10 is a grade 1 assessment (G1), so you would look for Form 10 in the booklets for grade 1 Passage Oral Reading assessments.
Mixed Format Assessments
After you choose the Mixed format, you'll see this message, which includes the form identifier (POR-G1: Form 09 in the example below). Find the student form with that ID in the booklet for the measure (in this case, "Passage Oral Reading Grade 1 Student Forms"). You do not need the teacher form since that will be shown in the software as you administer the assessment. When you have the student form, select Begin Assessment to continue. If you don't administer the assessment right away, you'll also see the form identifier when you select In Progress in the Record Book and then choose to start the assessment:

Print Assessments
After you choose the Print format, you'll see this message, which includes the form identifier (LN: Form 06 in the example below). Find the form with that ID in both the student and teacher booklets for the measure - in this case, "Letter Naming Student Forms" and "Letter Naming Teacher Forms." When you have the form, select Got It to go back to the Record Book. (Note: If you mark scoring information on the teacher form, you may need to reprint the teacher form each time.)

When you enter the score after the assessment, you will see the form identifier again in the popup:
